you share legal decision making authority (custody)
for a child in need of psychological services?
If you are calling
Dr. Garber about a child AND you have shared or joint legal custody with
another adult, the law requires that the other legal custodian(s) be alerted.
For example: A divorced mother shares legal custody and wants help with her
son. The father must be notified. Print, fill-in, copy and mail this letter.
Please bring a completed copy to Dr. Garber.

Co-parents' Mutual
Informed Consent
to release matters concerning a child's psychotherapy records. Both co-parents'
signatures are assumed
to be necessary unless legal documentation to the contrary is provided.

Children’s Rights
to Confidentiality as Psychotherapy Clients
This "Berg" acknowledgement may be necessary under
recent NH Supreme Court
rulings when a child's psychotherapy records cannot be released.

Authorization
to Release Protected Health Information
Print, complete and deliver this form to Dr. Garber
in order to authorize communication of Protected Health Information
(PHI) to individuals and agencies at your discretion.

Accounting
for communications of Protected Health Information
Print, complete and deliver this form
to Dr. Garber in order to request an accounting of any confidential communications
of your Protected Health Information (PHI) beginning 14 April, 2003.
Please note that while Dr. Garber has always
worked to respect patient confidences within the limits dictated by ethics
and law, commincations prior to 14 April, 2003, may not be subject to HIPAA
compliant accounting practices.
