Do you share legal decision making authority (custody)
for a child in need of psychological services?
If you are calling Dr. Garber about a child AND you have shared or joint legal custody with another adult, the law requires that the other legal custodian(s) be alerted. For example: A divorced mother shares legal custody and wants help with her son. The father must be notified. Print, fill-in, copy and mail this letter. Please bring a completed copy to Dr. Garber.

Therapy with Children of Conflicted Caregivers:
This is a generic version of the terms under which Dr. Garber may be available to provide psychotherapy to children caught in the midst of intense caregiver conflict, marital separation and/or divorce. Please keep in mind that psychotherapy and evaluation relevant to matters such as custody are quite distinct.

Confidentiality and communications. 
These forms are provided here for your convenience. Please be advised that Dr. Garber will require receipt of an original, hand-signed form. Electronic reprodictions and transmissions cannot be accepted.
Co-parents' Mutual Informed Consent
to release matters concerning a child's psychotherapy records. Both co-parents' signatures are assumed
to be necessary unless legal documentation to the contrary is provided.
